Welcome to our practice

New patient visits and follow-ups take place at our office at 488 E 11th Ave #150a in Eugene (get driving directions). We’re on the ground floor of the Oregon Community Credit Union building. Please bring:

• Any pertinent medical records
• Your most current insurance information and insurance card

Please arrive early to allow time for parking and finding our office. We do our best to accommodate every patient, but if you’re late for your appointment we may need to reschedule you for another day. If you miss your appointment without 24 hours’ notice, you may be subject to a $50 cancellation fee.

You may be asked to complete any or all of these forms, depending on the nature of your visit. Our office staff will let you know which ones you need. You can download them and fill them out ahead of time to bring to your appointment.

Insurance

Most insurances accepted (preferred / in-network): Aetna, Blue Cross, Cigna, PacificSource, Providence, MODA, and Medicare.

Out-of-network: Health Net, Kaiser, and UnitedHealthcare.

Insurances not accepted: all Medicaid plans and supplements.
